Basic Education Program
General Application Requirements

• Photocopy of Form 138 (if applicable)
• Photocopy of PSA Authenticated Birth Certificate
• Recommendation Form (Grade 3-10/JHS/SHS)
• Certificate of Good Moral Character (Grade 3-10/JHS/SHS)
• Recent digital colored photo

2 x 2 (2 copies)
• Application fee and entrance examinations are waived.
• Applicant must submit original documents to be eligible for admission.
Admission Procedure
• Secure an application form from the Office of Admissions. • Application and testing fee is Php 400.00.
• Fill-out and submit the completed application form, together with all other requirements, at the Office of Admissions.
• Pay the application and testing fee.
• Secure entrance examination schedule form.
• Take the entrance examination on the scheduled date.
• Pay the reservation fee. Reservation fee is non-refundable but is deductible from the total fees when enrollment is made official.
Entrance Examination
• Applicant must be online 30 minutes prior to scheduled examination.
• Present entrance examination permit for examination instructions.
• Examination results are released after five (5) working days.
The testing schedule is determined at the time of application. Latecomers will not be allowed to take the test; however, they may be rescheduled for testing.

PROFESSIONAL COURSES
General Application Requirements
Applicant must submit a copy of the following documents to be eligible for admission.
• Accomplished Student Application Form
• Two (2) pcs of recent passport size photo with white background
• Photocopy NSO Birth Certificate • Photocopy of NSO Certificate of Marriage, for applicants who changed their names after marriage
• Photocopy of two (2) Government issued identification card.
• Photocopy of passport, for International applicants
• A signed waiver form for those who have already booked an International English Exam.
Additional requirements (if Applicable)
• Photocopy of previous exam result, for the students who already took the exam.
• Photocopy of PRC license, for nurses’ applicants.
• Photocopy of Present School ID, for minor applicants
• Photocopy of Valid ID of Parent/Guardian, for minor applicants
• Accomplished Fetcher’s Form, for minor applicants.
